In my previous job, I had to make sure our data entry for patients match what is written in their charts. If something was unclear, I would ask for help if someone else wrote in the chart or when the patient comes back/we give the patient a call and ask to clarify. Everytime a chart is finished, I make sure the data is in right away after getting doctor signatures. The systems I used was Epic and Medidata.
